Wegovy and Arthritis: GLP-1 Found in Joint Fluid

Patients with arthritis who shed weight using medicines such as Wegovy often report improved joint symptoms. Clinicians have usually attributed this to weight loss alone: reduced pressure on cartilage, less wear and tear, and fewer flare-ups.

However, a small study from Denmark indicates that this may not be the whole story. Researchers identified the hormone imitated by these medicines in a location where it had never previously been recorded, a discovery that could affect understanding of how the drugs work.

GLP-1 in joint fluid

Joints rely on a dense fluid known as synovial fluid to cushion cartilage and enable smooth movement. Before now, GLP-1 - the hormone behind current weight-loss drugs - had not been documented in this fluid in living patients.

Researchers at Aarhus University in Denmark have now done so. The project was led by Tue Wenzel Kragstrup, M.D., Ph.D., an associate professor of biomedicine, alongside two Ph.D. students. Their aim was to establish whether the hormone reaches the joints at all.

Although this may appear straightforward, it had never been resolved. The answer alters how researchers consider a group of medicines already found in millions of medicine cabinets.

What the samples revealed

The samples were supplied by a Danish biobank for inflammation research. They included blood and joint fluid taken from the same patients, all of whom had rheumatoid arthritis or a related inflammatory disease. Blood samples from healthy volunteers were used for comparison.

Across the two groups, the researchers measured GLP-1 in 44 patients. They found the hormone in both groups. Its levels were so low that previous testing methods had failed to detect it.

No earlier study had identified GLP-1 within a human joint. Scientists had long considered its presence possible because of the hormone's functions elsewhere in the body, but this had not been demonstrated. The Danish researchers have now provided that evidence.

Weight-loss drugs and GLP-1 entering joints

The quantities recorded by the team were extremely small. Levels of GLP-1 in joint fluid were lower than those in blood, but the two measurements closely corresponded. The amount circulating in the bloodstream appeared to pass into joints at a consistent proportion.

This points to a particular conclusion: the body does not seem to produce GLP-1 within the joint itself. It is not made locally or as required. Instead, the hormone appears to enter from the bloodstream.

Weight-loss medicines alter this equation. Their doses raise blood GLP-1 to levels far above the body's natural production, with joint concentrations rising in turn. A recent review found evidence that these higher doses could directly lessen inflammation.

The DPP-4 enzyme issue

GLP-1 has only a short lifespan in the body. It is rapidly broken down by an enzyme called DPP-4, limiting how far the body's own hormone supply can act in any tissue. Modern weight-loss drugs are designed to withstand this enzyme's effects.

The Aarhus researchers measured DPP-4 in joint fluid as well and found that it was both present and active, ready to break down arriving GLP-1. This had not previously been documented in living patients with arthritis.

The result is important for anyone considering direct delivery of a GLP-1 medicine into a joint. A drug injected into a knee, for instance, would encounter the enzyme immediately. It would therefore need to be designed to survive that exposure.

Beyond weight loss

Doctors already advise many people with arthritis to lose weight, as excess body weight wears down joints and aggravates inflammation. A medicine such as Wegovy can provide reliable weight loss for people who find it difficult to achieve by themselves.

The Danish research indicates that the same medicine could be working in two ways. Increasing GLP-1 levels in the blood would probably drive more of the hormone into the joints, where modest anti-inflammatory effects may accumulate.

“Our study may indicate that medication such as Wegovy could have a dual effect – both through weight loss and by increasing GLP-1 levels in the joints,” Kragstrup said. A previous paper that reviewed the evidence had identified signals meriting closer investigation.

What happens next

The researchers are explicit about the scope of their work. They detected a hormone, rather than a treatment, and did not prove that GLP-1 medication treats arthritis. That remains an unanswered question requiring clinical trials.

According to Kragstrup, the next task is to verify that the medicine reaches joints in useful quantities and reduces inflammation there. Doctors will not prescribe these drugs specifically for arthritis until those results are available.

What has shifted is the question being asked. Prior to this study, scientists could not establish whether the hormone reached the joint in the first place. They now know that it does, as well as what obstructs it.

Researchers now have a defined target for testing the next generation of drugs. Whether weight-loss medicines genuinely reduce arthritis activity within the joint is now a question that can be investigated.
