A straightforward workout does far more than firm up muscles or use calories. Scientists can measure how it alters brain chemistry and improves mood.
A large scientific review by James Cook University confirms that physical movement delivers genuine mental health benefits.
Having assessed findings from numerous high-quality studies, the researchers concluded that regular exercise eases symptoms of depression and anxiety.
The improvements were seen in age groups ranging from teenagers to older adults, indicating that remaining active benefits the mind as well as the body.
How exercise was assessed
The research team conducted a meta-meta-analysis. Rather than considering a single trial, it brought together 63 major reviews.
Altogether, the analysis included 1,079 randomised controlled trials and 79,551 participants. Numbers on this scale make the findings more reliable.
The review included only studies involving structured, planned physical activity. Participants ranged from children to older adults. Those with serious long-term physical illnesses were excluded, allowing the results to focus clearly on mental health outcomes.
The researchers applied strict scientific standards and advanced statistical methods to establish the overall effect.
For depression, the mean effect size was minus 0.61. For anxiety, the average effect size was minus 0.47. Put simply, exercise led to meaningful improvements.
Who benefits most from exercise
Benefits were evident at every age. The largest effects, however, were found among emerging adults aged 18 to 30. Postnatal women also experienced substantial mood improvements.
Depression commonly starts during early adulthood, while the postpartum period brings a high risk of low mood. The strong response in these groups underlines exercise as a valuable form of early support.
Children and older adults improved too, although effect sizes differed slightly. Results were positive for every group included in the research.
Best types of exercise
Aerobic exercise had the greatest effect on both depression and anxiety. Running, fast walking, cycling and swimming are all examples of this type of activity.
Across many analyses, the effect size for aerobic activity and depression reached minus 0.81.
Resistance training, including strength exercises, also lowered symptoms. Mind-body activities such as yoga and tai chi improved mood too. Programmes that mixed different exercise styles were beneficial as well.
Group sessions had stronger antidepressant effects than exercise completed alone. Supervised programmes also performed better than unsupervised sessions.
Social support is likely to be important. Working towards shared aims and receiving encouragement may increase motivation and a sense of belonging.
The role of exercise intensity
For depression, moderate-intensity sessions frequently delivered strong benefits. Programmes extending beyond 24 weeks produced particularly large symptom reductions.
The pattern for anxiety was different. Low-intensity activity was more effective than moderate-intensity exercise.
Brief programmes of up to 8 weeks had a strong effect. Anxiety benefits were smaller in longer programmes lasting beyond 24 weeks.
These results indicate that programme design should reflect the mental health aim. Calmer, shorter sessions may be more suitable for anxiety, whereas longer, moderately demanding sessions may better suit depression.
How exercise changes depression
The review states that physical activity affects cognitive and neurobiological pathways. Exercise boosts the growth of brain neurotrophins, which help brain cells survive and develop. It also offers protection against neurotoxic damage.
Better sleep, improved cardiovascular health and greater self-confidence all contribute to emotional stability. In group environments, social contact may trigger psychological feelings of belonging and a shared identity.
The researchers also compared exercise with medication and psychotherapy. The effect sizes were similar.
This comparison does not mean that medication or therapy should be discontinued. Rather, the evidence indicates that exercise deserves equal consideration within treatment planning.
Limitations of the study
The researchers applied considerable care when selecting studies. They included only robust experiments in which participants were randomly assigned to groups.
This study design is regarded as one of the most dependable methods for testing an intervention. As a result, confidence in the findings is high. Most studies also did not reuse the same data, further strengthening their trustworthiness.
However, exercise was not defined identically in every study. Some used different descriptions of intensity and programme duration.
There were also fewer studies examining anxiety among children and older adults. Further research could clarify these areas.
Despite these limitations, the central finding remains clear: exercise helps to improve mental health.

Depression and anxiety affect millions of people around the world. In many areas, access to therapy or medication may be restricted. Exercise is inexpensive and brings additional physical health benefits.
A walk with friends, a dance class, weightlifting at the gym or yoga at home can all help support emotional recovery.
